Affordable Connectivity Program Opt-In

The Affordable Connectivity Program is a temporary FCC program to help households pay for internet service. If you’re eligible, this benefit will provide a monthly credit up to $30 per month, or up to $75 in Tribal lands, toward your broadband service bill.

To apply for the Affordable Connectivity Program monthly credit, please fill out the form below. Be sure to select Submit Application to apply for eligibility in this program.

A member of your household must qualify for either the Lifeline Program or the Affordable Connectivity Program to participate.

Have you applied to the National Verifier yet? If not, complete your eligibility check https://acpbenefit.org before you attempt to submit this form. You must receive their approval before you can be accepted into this program.

Once your application is processed you will receive confirmation of your enrollment or if further information is needed.

    PROGRAM DISCLOSURES AND CONSENTS
    1. The Affordable Connectivity Program (ACP) is a government program that reduces the customer’s internet service bill;

    2. You will be subject to TWN’s applicable rates and general terms and conditions if/when:
    o ACP ends;
    o You transfer your ACP credit to another provider but continue to receive service from TWN,
    o You are no longer enrolled in ACP;

    3. You may obtain internet service with the ACP credit from any participating provider;

    4. You may file a complaint against your ACP provider regarding your internet service credit via the FCC’s Consumer Complaint Center;

    5. If you are transferring your ACP credit to TWN, you understand:
    o Your ACP credit will be applied to TWN Internet service and not to your other service(s);
    o That if you have service(s) with another provider, you may be subject to that provider’s undiscounted rates; and
    o Your household is limited to one ACP transfer transaction per calendar month with limited exceptions

    6. You agree to allow TWN to share your personal information with the Universal Service Administrative Co.'s National Lifetime Accountability Database (NLAD) to confirm your eligibility to participate in the EBB Program.

    7. These terms apply to all TWN customers whether or not they are eligible for ACP and participate in the program. An eligible household may apply the ACP credit to any TWN Internet service at the same terms available to households not eligible for the ACP service credit.

    8. TWN may disconnect the household’s ACP-supported service after 90 consecutive days of non-payment.
